One Example is;
A newspaper report stated that Virginia McNamara, was a nurse who failed to report an abused 10-week old infant, Ethan Henderson, who consequently died. Ethan Henderson was abused by his father, Gordon Collins-Faunce.
McNamara was accused of failing to report a broken arm and bruises to the Maine Department of Health and Human Services and lost her nursing license. His father subsequently pleaded guilty to manslaughter and the courts sentenced him to 20 years imprisonment.
The failure to report caused the father to continue to physically abuse him, which inevitably caused the infants life.
